چکیده

Spread tumors in liver are not suitable to be treated with local treatments, such as conventional surgery or radiofrequency ablation, thus entailing a poor prognosis. Electroporation-based therapies imply the delivery of pulsed high electric fields and currently are performed in a local fashion using needle or plate electrodes. Here, however, it is proposed a novel electroporation paradigm in which field delivery is not local. All the tumor nodules will be selectively treated using large plate electrodes at both sides of the liver. By infusing an hypersaline solution of high electrical conductivity through the portal vein, the electrical conductivity of healthy tissues and tumor nodules will be made significantly different so that the electric field will be focused on the undesirable tissues. Numerical simulations were used to evaluate the feasibility of the proposed technique. In addition, an in vivo procedure was carried out to assess whether it is possible and practical to significantly modify the conductivity of the liver tissue by hypersaline infusion. Both the numerical simulations and the in vivo procedure provided encouraging results. Keywords— Electroporation, Liver, Spread tumors, Disseminated nodules, Hypersaline.

Focused Transhepatic Electroporation Mediated by Hypersaline Infusion through the Portal Vein in Rat Model. Preliminary Results on Differential Conductivity

Background Spread hepatic tumours are not suitable for treatment either by surgery or conventional ablation methods. The aim of this study was to evaluate feasibility and safety of selectively increasing the healthy hepatic conductivity by the hypersaline infusion (HI) through the portal vein. We hypothesize this will allow simultaneous safe treatment of all nodules by irreversible electroporat...

Modeling liver electrical conductivity during hypertonic injection.

Metastases in the liver frequently grow as scattered tumor nodules that neither can be removed by surgical resection nor focally ablated. Previously, we have proposed a novel technique based on irreversible electroporation that may be able to simultaneously treat all nodules in the liver while sparing healthy tissue. The proposed technique requires increasing the electrical conductivity of heal...

Ex Vivo and In Silico Feasibility Study of Monitoring Electric Field Distribution in Tissue during Electroporation Based Treatments

Magnetic resonance electrical impedance tomography (MREIT) was recently proposed for determining electric field distribution during electroporation in which cell membrane permeability is temporary increased by application of an external high electric field. The method was already successfully applied for reconstruction of electric field distribution in agar phantoms. Before the next step toward...

A Multi Objective Genetic Algorithm (MOGA) for Optimizing Thermal and Electrical Distribution in Tumor Ablation by Irreversible Electroporation

Background: Irreversible electroporation (IRE) is a novel tumor ablation technique. IRE is associated with high electrical fields and is often reported in conjunction with thermal damage caused by Joule heating. For good response to surgery it is crucial to produce minimum thermal damage in both tumoral and healthy tissues named Non-Thermal Irreversible Electroporation(NTIRE). Non-thermal irrev...

Electroporation-Based Treatment Planning for Deep-Seated Tumors Based on Automatic Liver Segmentation of MRI Images

Electroporation is the phenomenon that occurs when a cell is exposed to a high electric field, which causes transient cell membrane permeabilization. A paramount electroporation-based application is electrochemotherapy, which is performed by delivering high-voltage electric pulses that enable the chemotherapeutic drug to more effectively destroy the tumor cells. Electrochemotherapy can be used ...
